To: Chan Kah Yein <chankahyein@gmail.com>Write up:Kelly was one of the strays at my block where I live.Me & some other residents have been feeding her for quite some time.She has been pregnant & gave birth for a few times. Most of her kittens either did not make it, or have been adopted. So it’s just her surviving alone, day by day.On February 2015 I brought her back home & she has safely gave birth to healthy five kittens (The Cekodoks). Kelly has been neutered & ready to be adopted. I have also advertise her on Petfinder at http://www.petfinder.my/pets/68461/ Her kittens are also available for adoption at http://www.petfinder.my/pets/68462/ Dear Dr. Chan,1) I would like to request for subsidy for her neutering. The receipt should arrive to you in a few days.She has also undergone some teeth removal at the back of her jaw as advised by the vet due to some growth. Unfortunately I am unable to give you the photos inside her jaw because the vet suggested to do it after I sent her for the neutering. But it has been stated in the same receipt as the neutering. I appreciate it if you can consider this surgery to be subsidized as well.2) I would also like to request for subsidy for her (five) kittens’ vaccination.But I was unable to find the form in your website. I was wondering if you are no longer subsidizing vaccinations.
