Please ensure the subsidy form is filled up completely before posting it to us

We would like to make another appeal to all applicants to PLEASE fill up the forms correctly, stating the type of animal and gender and also to fill up their bank account numbers correctly and completely before submitting the form to us. Please understand that we receive many applications in a day and have to process them, and pay out subsidies (the bank transaction can only be carried out successfully if the bank account no is correct). The applicant only has ONE form to fill. If applicants could kindly ensure that all details are filled up correctly before posting the forms, it would save us so much time and energy. Your cooperation on this matter is highly appreciated.
