How to apply for a subsidy, fill up our forms, label photos, etc.

To apply for a subsidy:

A summary:

1. Email jpeg photos of the animal before and after treatment/procedure. Email a short write-up of the illness/procedure/treatment.
2. Post the form and original receipt to reach us within 2 weeks (14 days) from the date on the receipt.

About the receipt:

Please ensure that:

a. The receipt is issued IN YOUR NAME.
b. The receipt is ITEMISED.

We’ve had a request from an applicant to provide a sample of how to fill up our forms. This may benefit some applicants who may need some pointers too.

To fill up our forms:

Use the correct form: (a) Neutering Form for neutering claim (b) Medical Form for medical and vaccination claims.

Write the NAME of your animal, for eg. Susie.
Circle Dog/Cat/Others.
Circle Male/Female.

Or, on the Neutering Form, write the number of animals (type and gender). You may also circle if it is just one animal.

Write the RECEIPT NUMBER.

Write the NAME OF THE VET’S CLINIC.

If there is no receipt number, you may leave it blank.

Write your bank ACCOUNT NUMBER.

Circle the NAME OF YOUR BANK.  (This is very important)

Write the name of the ACCOUNT HOLDER (which should be your IC name).

Answer the question YES or No.

If you are also applying to another organisation, write the name of the organisation you are also claiming from.

CHECK all details and SIGN the form.

Write your PROFESSION, IC NUMBER, HANDPHONE NUMBER, EMAIL ADDRESS and write the DATE.

If you are predisposed to being careless, please get a friend or relative to check the form. Reason: If your form reaches us incomplete, we would have to ask you to resubmit within the deadline. This would inadvertently cause a delay.

Post the duly completed form together with the original receipt. Use a standard-sized envelope (white, measuring approx 24cmX11.4cm) with a minimum of 60 sen stamps OR use Pos Ekspres to ensure next day delivery.

Check your email for our payout. If all your documentations are in order, we pay out within 24 hours AFTER receiving your form and receipt.

After receiving our subsidy, kindly reply the email to say you have received it.

Although this is NOT a requirement, but if you would like to help me, I would truly be most grateful if you could label your photos with your animal’s name and a short description. For this, there has been a request for pointers on how to rename photos:

How to rename/relabel a photograph:

Point your mouse at the photograph and RIGHT-CLICK.
A drop-down box will appear.

Your cooperation is much needed and very much appreciated because it would save us a lot of time in processing your email, photos, etc. and doing all the paper work for each application.

Some of you who have not complied with the requirements would have received emails from the exco requesting further information or action.

This is how we work:

1. Your emails to me are forwarded to the exco.
2. Your application form and receipt are received by the exco, vetted and approved by them. Therefore, please ensure that the receipt is ITEMISED. We will not be able to subsidise if your vet lumps everything into one item without any description.
3. If you have complied with all requirements, your application is forwarded to me and you will be paid within 24 hours.
4. If you have not complied with all requirements, the exco will write to you for further action and this will cause a delay in the whole process.
