We have subsidised RM150 for the spaying of Dolly. This subsidy comes from the AnimalCare Donors (RM50) and Berjaya Cares (RM100).

I would like to request for your kind consideration to subsidise the neutering of a female dog named Dolly under my care.
Dolly was rescued in July 2013. She was around 10 weeks old at that time. I took her in as she was too young to fend for herself and she suffered mild skin problem. I have tried to rehome her however no interested adopter come forward for her.
I have recently spayed her as she has reached her maturity of 7 months.
I have read the terms & conditions as published in animalcare website and confirmed my acceptance to all the requirement as stated there. The fee of RM250 was fully paid by me and I didn’t receive any donation from public.I enclose the photo of Dolly taken at the vet and a copy of the bill.
