Vaccination subsidy for 3 dogs (Suan Choy Yong’s)

We have subsidised RM75 for the vaccination of these 3 dogs.

Good morning to you

Just to recap, female stray pup A (now name Goofy) & female stray pup B (now nameEger) were rescued on 7 Dec 2013 and both sent for 1st vaccination on 21/12/2013.

This attached subsidy vaccination claim is meant for their 2nd vaccination (vaccinated on 21/01/2014 at USJ Animal Clinic)

I have also rescued a pup (abandoned at PJ Thai Temple) on 17/01/2014
I named this pup Angel, and claim her 1st vaccination (vaccinated on 21/01/2014)
