Vaccination subsidy for 2 dogs, Baby Blue & Pinky (Carol Ng Siew Theng’s)

We have subsidised RM50 for the 1st vaccination of Baby Blue and 2nd vaccination of Pinky.

Condition before & after vaccination: Healthy

Other details: Pinky was stray puppy found at USJ1, currently being fostered by me and put up for adoption at the following link: http://www.petfinder.my/pets/53759/. To date, no one has contacted me with interest to adopt her, most likely due to her leg injury which is still healing. Her 1st vaccination was done on 11 March 2014.

I am submitting her form & receipt together with that of Baby Blue. Thank you!
