My name is Subashni A/P Bahsu – once again im writing to request for subsidy from animal care.
This subsidy request is for the vaccination cost for “Momo”.
Attached are her photographs, vaccination sticker pict & copy of the receipt for your reference.
Story of Momo;
Momo is one of the puppies of Mummy (a neighbourhood stray – which is already spayed now –http://animalcare.my/2014/03/20/spaying-subsidy-for-mummy-female-dog-in-johor-subashni-bahsus/).
Momo is lucky that she has a warm fosterer taking care of her while we look for an adopter. Chances are higher to get adopters if the animal is vaccinated & spayed. Momo has a profile created on petfinder for brighter luck in adoption.
This application is to seek subsidy for the vaccination cost of Momo.
NOTE:
There are still 3 more puppies left from Mummy’s last litter. Am doing vaccination in badges.
I will submit their form once the vaccination is done.
