1st vaccination subsidies for 2 cats and 2 dogs (Lyzz Sew Shew Lai’s)

We have subsidised RM100 for these 4 vaccinations (for Bell, Tomy, Blossom and Sunshine).

Dear Dr Chan,

I am writing to apply vaccination subsidies for the following animals.

3 months ago a stray female cat that I feed gave birth to 4 kittens.

They are named Tin Tin, Felix, Tomy and Belle.

I have successfully apply vaccinations subsidies for Tin Tin and Felix.

Now I would like to apply for Tomy and Belle.

About 2 weeks ago I took 2 puppies that was dumped at my guard house.

Both are females aged 3 months

I named them Sunshine and Blossom and i have sent them for vaccinations.

I am trying to rehome all the above animals.

Hoping my application will be successful.

The receipts posted by pos ekspress.

FYI I have received the subsidies for the amount of $175.
