I have brought a new cat, Tommy for neuter at the clinic on Monday, 12th August 2014 and I would like to apply for AnimalCare’s subsidy for the procedure done.
Tommy came to my apartment last September when he was approximately 2 months old. He is well taken care by me and other residents J
I am pleased to enclose here the completed subsidy form, receipt and photos of Tommy before and after the procedure for your consideration. I will put the original document to your postbox by next week.
