I have posted the original receipt together with the duly completed form today. Both should reach you within this week or early next week.
My parents rescued her in front of our house. Someone abandoned her with her puppies too. We have re-homed all the puppies and left only her to take care due to her skin problem.
We will re-home her after she recover from her skin problem.
