Dear Dr Chan

This is some write up on Blurr.

Blurr was born by a stray cat at my house somewhere in May 2014. They were four but only two survived named Jumbo (boy) and Blurr (girl). Both have been adopted by me.  Blurr was neutered on November 28, 2015 with total cost of RM159.The copy of the bill and photo of  Blurr are attached for your reference. For your  information, in Feb 2015, I made a subsidy claim for Jumbo.

Since I have the link to your application form, I will fill up the necessary and post ekspress it today.
