I am applying the above for this cat named Medan.  He was found loitering on the street with chronic sore eyes, flu and very skinny somewhere in July 2015 and was about 3 months aged.  Since then he becomes my family.  The castration was done on December 30, 2015.  The copy of the receipt and photo are attached for your reference.
