Good day to you.

I hereby submit my application for neutering subsidy for 1 female stray cat.

The cat was found at my backyard, malnourished, underweight and not in a good health. I’ve adopted this cat and named her Pogo.

I do not have the before photo as Pogo is not really in good health and I brought her to several visit to vet clinic to check on her health condition. But I have her very young photo (labeled “before”).

Attached herewith is the receipt from the vet clinic and also the after photo.

Please do not hesitate to ask me any question regarding Pogo.
