Neutering aid for 3 cats in Mentakab (Fadziliyaton bt Lajim’s)

We have sponsored RM180 for the neutering of these 3 cats.

I am applying subsidy for the following stray cats under my care named:

1. Joya (estimated 8m)

2. Che Ta (estimated 8m)

3. Tami (estimated 1.5y)

They have undergone the surgery on July 12, 2017 at a Mentakab clinic.  Attached herewith photos of these cats for your reference.

I will send to you by post express the original receipt of the operation together with the completed form for your further action.
