This is my second time applying for neutering aid.
I would like to apply for the neutering aid for a female brown dog, named Meredith. As per my knowledge, she has given birth twice before. She is one of the many stray dogs from a nearby orchard in Air Putih, Balik Pulau (Penang). She comes by my house backlane for evening meals. Recently I was able to catch her and spayed her on 15 November with 2 nights vet accommodation. Her ear has been notched according to Majlis Bandaraya Pulau Pinang specification. I have released her back to the community after she has recovered.
Attached are my neutering aid form, pictures and receipt for your kind consideration. Do let me know if you need further details. I will send the documents to you via Pos Express as per the usual requirement.
p.s. My first neutering aid from AnimalCare was in July for Meredith’s daughter, Olivia. Olivia is now doing fine, playful and quite chubby.Thank you and best regards,
