Dear Dr Chan,

As we are feeding a colony of cats & kittens, an outsider came meowing loudly into our back lane about a week ago, looking very thin and having a very bad flu.  At that time, I thought it was a male as it was quite a big cat and dare not go near it as I was afraid that it might bite. However, after a few days of giving it food and water, I could pat and stroke it and to my surprise it was a female.

It was friendly towards the others and would want to share their food so each time when it came, we had to feed it further away so that it will not spread the flu to the others.

Last Saturday, I brought it to the Vet for treatment, he found her to be at least 7 to 8 years old with some missing teeth and he has agreed to treat her at his clinic for a few days.

On this note, I would like to apply subsidy for her medical treatment and hope to receive a favourable reply.
Regards,
Veronica

Dear Dr Chan,

We took her back home from the clinic this evening and immediately she walk out from the carrier, look around and walk straight into the back lane without looking back.  Must be very afraid.
We have yet to decide a name for her so we temporarily called her “Jane Doe”.
As required, attach please find the duly signed & completed Medical Subsidy Form  and receipt no. 16868 amounting to RM90.00 for flu treatment/medication for your records.
The original copies will be sent to you via postal mail.
Thank you and kind regards,
Veronica
