We have subsidised RM242.50 for the surgery and medical treatment of Coby, a cat rescued by Mohd Fazrul Azuani. Amputation of the front leg was performed and Coby was also castrated at the same time. This subsidy comes from all donors through our Medical Fund (RM102.50) and Berjaya Cares (RM140.00).

Subject: coby the three legged cat
To: chankahyein@gmail.comDear Dr ChanI am Mohd Fazrul Azuani Bin MazlanI would like to apply for subsidy for Coby the cat.
Coby the cat was found at Bukit Gantang rest area, Perak when I was there. The cat was limping and came to me and showed me his leg which was wounded as in the picture. I pity the cat and decided to take him to KL . I contact a kind person who took the cat immediately to clinic Mayo. I could not do anything as I was on the way to Singapore as a bus crew. The kind person immediately bring the cat to clinic and the vet advised that the operation had to be done for the cat. The wound was bad.
So Coby (I named him) was operated and neutered.
Now Coby is fine and operated and eating very well.
Coby had been a stray in the Bukit Gantang area and was very smelly and dirty and the doctor and staff cleaned and shower him before operation.
I would like to apply subsidy for Coby and will attach my receipt for Coby. Coby will be under foster by kind person until I can find a lovely home for him.
Thanks Dr Chan.
Mohd Fazrul Azuani bin Mazlan
