Hope you are well and in good health.Margie and I would like to apply for subsidy for Candy, a young female cat and Trixie a female dog .Candy is a stray from the Bandar Utama area and Trixie is from the Orang Asli settlement area.They have both been spayed and are doing well.The veterinary clinic has issued one receipt for both animals and for clarity Candy’s procedure cost rm169.60 and Trixie’s cost rm265.We have posted the receipt and form.Thank you very much for your kind assistance.Warm regardsSusan
