I am humbled to ask for your consideration to apply for subsidy for neutering 1 female kitten.
Memei was dump with few other cats in front of a pet shop in Kuantan two months ago. She had severe flu where the vet had her eyes removed since the flu spread and affected her eyes. Memei was spayed yesterday. She is now living with me and I will continue to take care of her forever.
Hereby I attached Memei’s photo for your perusal. I will courier the receipt together with the claim form by tomorrow.
