Making it simpler for applicants (NEW forms!)

We have simplified our forms for all applicants!

Kindly download our new forms if you wish to apply for neutering or medical subsidy.

 We’ve changed it to a table so that you only have to fill in the no of animals in the correct column.

If you happen to rescue a rabbit, tortoise, etc., it comes under “Others”. Kindly state the type of animal in your correspondence with us.

As to whether you are sourcing other funds, please answer Yes/No, and if Yes, kindly state the name of the organisation or source(this includes sourcing funds through Facebook).

We really hope our new form will be easier for everyone to fill up the required details.

    Is the subsidy applicable to animals other than cats and dogs?

    If it is, but the form only contains cats and dogs, then some people with other animals may not realise they can apply. And some of these people might actually need financial assistance (those who can afford it shouldn’t be applying in the first place).

      We have not received any claim for other animals so far. If there are any, the applicant can always write in.

        Thanks to Martin, I have added an “Others” column for other species (rabbits, iguanas, etc.)

    Hi KY,

    Feedback regarding the “table” format used. I’m not sure if it’ll make the form-filling easier for your target group. I know of intelligent adults who cannot understand anything the moment they see info in a table or chart format. It’s very simple for people who are good with math and logic to understand this kind of info presentation, but not for other kinds of thinking-types. I call it chart-table dyslexia.
