Vaccination subsidy for 2 TNRM cats (Koo Swee Por’s)

We have subsidised RM40 for the two vaccinations and RM9 for the deworming of these 2 cats, managed by Koo. This subsidy comes from all donors through our Medical Fund.

Hi Dr Chan,

Thank you very much to AnimalCare for agreeing to provide neutering
and medical subsidies for the following cases:

Cheekie & PC (pictures 01-02)
Both are siblings who have been spayed/neutered (with AnimalCare
subsidy too) on 23 May 2013. On 5 June 2013, I’ve dewormed and
vaccinated them and would like to apply for a subsidy.

The completed application forms, the original receipts and copies of
the vaccination cards have been posted to AnimalCare.
