Dear Applicants,
We wish to inform you that with effect from today, we require the fully itemised printed receipt (with your full name) when you apply for our neutering subsidies.
If the printed receipt has no name on it, please request that the vet writes your full name on the receipt and this must be stamped and signed by the vet.
If the printed receipt is not itemised, please produce a note, signed and stamped by the vet, that clearly says how much has been charged for neutering.
