Vaccination sponsorship for May the Dog and Mimi the Cat (Lyzz Sew Shew Lai’s)

We have subsidised RM50 for May’s first vaccination and Mimi’s 2nd vaccination.

Dear Dr Chan,

I am writing this email to apply vaccinations subsidy for Mimi and May.

I rescued May the female puppy 3 weeks ago near my guard house and she is about 2 months old now.

My friend is fostering her while I try to rehome her.

Attached are photos and vaccination cards for the animals.

I hope my application will be successful.
